Objetivo del estudio:

  • Evaluar los resultados a largo plazo del alotrasplante facial.
  • Evaluar los puntos finales funcionales, inmunológicos, metabólicos y de integración social.
  • Para determinar la relación riesgo-beneficio del trasplante de cara.

Principales métodos:

  • Estudio prospectivo, abierto y monocéntrico de siete pacientes con trasplante facial.
  • Se informó un seguimiento a largo plazo (3,4-9 años) de seis pacientes supervivientes.
  • Datos recogidos sobre eventos adversos, rechazo, efectos secundarios inmunosupresores, calidad de vida (SF-36) y integración social.

Principales resultados:

  • Seis de los siete pacientes recibieron trasplantes funcionales en el seguimiento a largo plazo.
  • Las reacciones adversas incluyeron infección, rechazo agudo (hasta 7 años) y efectos secundarios inmunosupresores (hipertensión, reducción del GFR).
  • La calidad de vida y la integración social variaron, influenciadas por factores de referencia y comorbilidades psiquiátricas.

Conclusiones:

  • El trasplante facial ofrece beneficios funcionales pero conlleva riesgos significativos a largo plazo.
  • El apoyo social del paciente y las condiciones psiquiátricas preexistentes tienen un impacto crítico en el equilibrio riesgo-beneficio.
  • La selección cuidadosa del paciente y el seguimiento a largo plazo son esenciales para futuros procedimientos de trasplante facial.
