Tumor Immunotherapy

Immunotherapy is a treatment that boosts or manipulates the immune system to fight diseases, including cancer. For instance, by stimulating an immune response through vaccinations against viruses that cause cancers, like hepatitis B virus and human papillomavirus, these diseases can be prevented. Nonetheless, some cancer cells can avoid the immune system due to their rapid mutation and division. The immune response to many cancers involves three phases: elimination, equilibrium, and escape.

Resumen

Lartruvo (olaratumab) es un anticuerpo dirigido al PDGFRA que mejoró la supervivencia cuando se añadió a la quimioterapia con doxorrubicina en un estudio de Fase II. Este anticuerpo inhibe la actividad de la cinasa PDGFRA, lo que es prometedor en modelos de cáncer preclínicos.

Sus antecedentes:

  • El receptor alfa del factor de crecimiento derivado de plaquetas (PDGFRA) está implicado en el crecimiento tumoral.
  • La orientación hacia el PDGFRA presenta una estrategia terapéutica potencial en oncología.

Objetivo del estudio:

  • Evaluar la eficacia y seguridad de olaratumab, un anticuerpo monoclonal dirigido al PDGFRA.
  • Evaluar el beneficio de la adición de olaratumab a la quimioterapia con doxorrubicina en pacientes con sarcoma avanzado de tejidos blandos.

Principales métodos:

  • Se realizó un estudio clínico aleatorizado de fase II.
  • Los pacientes recibieron doxorrubicina sola o doxorrubicina junto con olaratumab.
  • La supervivencia global fue un objetivo primario.

Principales resultados:

  • La adición de olaratumab a la doxorrubicina mejoró significativamente la supervivencia general en comparación con la doxorrubicina en monoterapia.
  • Olaratumab demostró un perfil de seguridad manejable cuando se combina con doxorubicina.
  • Los estudios preclínicos mostraron la inhibición del crecimiento tumoral dependiente del PDGFRA.

Conclusiones:

  • Olaratumab en combinación con doxorrubicina ofrece un beneficio en la supervivencia de los pacientes.
  • Olaratumab representa un nuevo enfoque terapéutico dirigido a la vía PDGFRA en el tratamiento del cáncer.
  • La aprobación de la FDA se concedió en base a estos hallazgos.
