Sus antecedentes:

  • El tratamiento contemporáneo del infarto de miocardio por elevación del segmento ST (STEMI) se basa en la intervención coronaria percutánea primaria (PCI).
  • Se están investigando terapias auxiliares para reducir aún más el tamaño del infarto en pacientes con STEMI sometidos a PCI.
  • La N-acetilcisteína (NAC), un antioxidante, potencia la nitroglicerina y es una terapia auxiliar potencial para el STEMI.

Objetivo del estudio:

  • Evaluar la eficacia de la N-acetilcisteína (NAC) como terapia auxiliar en la reducción del tamaño del infarto en pacientes con infarto de miocardio por elevación del segmento ST (STEMI) sometidos a una intervención coronaria percutánea primaria (PCI).

Principales métodos:

  • Un ensayo multicéntrico aleatorizado, doble ciego, controlado con placebo (NACIAM) en el que participaron 112 pacientes con STEMI sometidos a ICP primaria.
  • Los pacientes recibieron dosis altas de NAC por vía intravenosa (29 g durante 2 días) o placebo, ambos con dosis de fondo bajas de nitroglicerina.
  • El tamaño del infarto se evaluó utilizando imágenes de resonancia magnética cardíaca temprana (CMR).

Principales resultados:

  • De 75 pacientes que se sometieron a una RMC temprana, los asignados al azar a la NAC mostraron una reducción absoluta del 5,5% en el tamaño del infarto en comparación con el placebo (11,0% frente a 16,5%, P=0,02).
  • El rescate miocárdico mejoró significativamente en el grupo NAC (60% frente a 27%, P< 0,01).
  • Las áreas medianas de creatina quinasa bajo la curva fueron más bajas en el grupo NAC (22.000 UI·h frente a 38.000 UI·h, P=0,08).

Conclusiones:

  • La administración de dosis altas de NAC por vía intravenosa en combinación con dosis bajas de nitroglicerina por vía intravenosa reduce significativamente el tamaño del infarto en pacientes con STEMI sometidos a PCI primaria.
  • Los hallazgos sugieren un beneficio potencial de NAC como terapia auxiliar en el tratamiento de STEMI.
  • Se requiere una investigación adicional en un estudio más amplio para confirmar el impacto en los resultados cardíacos clínicos.
