Sus antecedentes:

  • El D-dimer, un producto de la degradación de la fibrina, indica hipercoagulabilidad y riesgo trombótico.
  • El aumento de D-dimero está relacionado con eventos arteriales y venosos en pacientes con enfermedad vascular.

Objetivo del estudio:

  • Evaluar el papel del D-dimero en la predicción de los resultados vasculares a largo plazo, la mortalidad y la incidencia del cáncer.
  • Evaluar el valor predictivo del D-dimero en el contexto de otros factores de riesgo dentro del ensayo LIPID.

Principales métodos:

  • Se analizaron los niveles basales de D-dimero en 7863 pacientes del ensayo LIPID, agrupados por cuartil.
  • Los pacientes fueron seguidos durante una mediana de 6 años durante el ensayo y 16 años en general.
  • Ajustado por hasta 30 factores de riesgo para determinar el valor predictivo independiente.

Principales resultados:

  • Los niveles más altos de D-dimero se correlacionaron con un mayor riesgo de eventos coronarios importantes, enfermedades cardiovasculares (ECV) y tromboembolismo venoso dentro de los 6 años.
  • El D-dimero predijo de forma independiente la mortalidad por todas las causas, la mortalidad por ECV, la mortalidad por cáncer y la mortalidad no por ECV durante 16 años.
  • El D-dimero también predijo un aumento de la incidencia de cáncer y una mejora en la reclasificación del riesgo de mortalidad y tromboembolismo venoso.

Conclusiones:

  • Los niveles de D-dimero son predictores independientes significativos de eventos arteriales y venosos a largo plazo, mortalidad por ECV y mortalidad por enfermedades no cardiovasculares.
  • El D-dimer es un predictor significativo de la incidencia y la mortalidad por cáncer, con asociaciones que se extienden más allá de los 10 años.
  • Estos hallazgos apoyan la asociación de D-dimer con eventos fatales en múltiples enfermedades.
