La canagliflozina redujo significativamente la muerte cardiovascular o la hospitalización por insuficiencia cardíaca en pacientes con diabetes tipo 2. Los beneficios de este inhibidor del cotransportador de glucosa de sodio fueron mayores en las personas con antecedentes de insuficiencia cardíaca.

Sus antecedentes:

  • Canagliflozin es un inhibidor del cotransportador de glucosa de sodio estudiado para la reducción del riesgo cardiovascular.
  • La insuficiencia cardíaca (IC) y la muerte cardiovascular son preocupaciones significativas en pacientes con diabetes mellitus tipo 2 (DMT2).

Objetivo del estudio:

  • Evaluar los efectos de la canagliflozina en la insuficiencia cardíaca y la muerte cardiovascular en pacientes con DM2.
  • Evaluar si los beneficios difieren según el historial basal de insuficiencia cardíaca.

Principales métodos:

  • El Programa CANVAS incluyó a 10.142 participantes de alto riesgo de DM2 asignados aleatoriamente a canagliflozina o a placebo.
  • El seguimiento tuvo una media de 188 semanas, siendo el objetivo primario la muerte cardiovascular o la hospitalización por HF.
  • Los análisis de subgrupos incluyeron participantes con y sin antecedentes de HF al inicio del estudio.

Principales resultados:

  • La canagliflozina redujo el compuesto de muerte cardiovascular o hospitalización por HF (HR 0,78; IC del 95% 0,67-0,91).
  • También se redujeron las HF fatales o hospitalizadas (HR 0, 70) y las HF hospitalizadas solas (HR 0, 67).
  • Una interacción estadísticamente significativa (P=0,021) indicó un beneficio potencialmente mayor para la muerte cardiovascular o la hospitalización por HF en pacientes con antecedentes de HF (HR 0,61) en comparación con aquellos sin (HR 0,87).

Conclusiones:

  • La canagliflozina demuestra eficacia en la reducción de la muerte cardiovascular o la hospitalización por HF en diversos subgrupos de DM2.
  • Los beneficios cardiovasculares del medicamento pueden ser más pronunciados en pacientes con antecedentes iniciales de insuficiencia cardíaca.
