  • El ensayo REPRISE III es una comparación multicéntrica y aleatoria de los dispositivos TAVR de expansión mecánica Lotus y CoreValve de expansión automática.
  • Este análisis se centra en la hemodinámica valvular derivada de Doppler y los resultados a 1 año en pacientes de alto riesgo.

Objetivo del estudio:

  • Evaluar rigurosamente la hemodinámica valvular derivada de Doppler y su impacto en los resultados clínicos a 1 año para los sistemas TAVR de Lotus y CoreValve.
  • Para comparar el tamaño de la válvula, el gradiente medio, el área de la válvula aórtica y el índice de velocidad Doppler entre los dos dispositivos TAVR.
  • Evaluar la relación entre los parámetros hemodinámicos y los resultados clínicos, incluida la fuga paravalvular.

Principales métodos:

  • 912 pacientes con estenosis aórtica extrema o de alto riesgo fueron asignados al azar (2:1) a Lotus (n=607) o a CoreValve (n=305).
  • Los ecocardiogramas transtorácicos con Doppler se realizaron hasta 12 meses después del implante y fueron analizados por un laboratorio central.
  • Se evaluaron los parámetros hemodinámicos clave y la fuga paravalvular, junto con los resultados clínicos.

Principales resultados:

  • CoreValve mostró inicialmente gradientes más bajos y un mayor área de la válvula aórtica / índice de velocidad Doppler que Lotus; las diferencias disminuyeron en 1 año.
  • Lotus demostró tasas significativamente más bajas de fuga paravalvular moderada/ severa durante el seguimiento (P< 0,05).
  • Las diferencias hemodinámicas no se traducen en variaciones significativas en la mortalidad por cualquier causa, accidente cerebrovascular incapacitante o calidad de vida a 1 año.

Conclusiones:

  • Lotus TAVR resultó en una mayor libertad de fugas paravalvulares moderadas / severas, pero tenía un área de válvula más pequeña y gradientes más altos en comparación con CoreValve.
  • A pesar de las diferencias hemodinámicas observadas, no hubo asociaciones significativas con el criterio de valoración compuesto de mortalidad, accidente cerebrovascular o fuga paravalvular moderada al año 1.
  • Los resultados clínicos y la calidad de vida fueron similares entre los sistemas TAVR Lotus y CoreValve a un año de seguimiento.
