Sus antecedentes:

  • Las ecuaciones actuales de predicción de riesgo de enfermedades cardiovasculares (ECV) a menudo utilizan cohortes obsoletas con diversidad socioeconómica y étnica limitada.
  • Las poblaciones contemporáneas de atención primaria difieren significativamente de las cohortes históricas utilizadas para el desarrollo de la ecuación de riesgo.

Objetivo del estudio:

  • Desarrollar y validar nuevas ecuaciones de predicción del riesgo de ECV utilizando una cohorte de Nueva Zelanda representativa a nivel nacional.
  • Comparar el rendimiento de estas nuevas ecuaciones con las ecuaciones recomendadas por los Estados Unidos (ecuaciones de cohorte agrupadas - PCE).

Principales métodos:

  • Se estableció una cohorte prospectiva de 401.752 pacientes de atención primaria de Nueva Zelanda (entre 30 y 74 años, sin enfermedad cardiovascular previa).
  • Los nuevos modelos de regresión de Cox incorporaron predictores clínicos, privación socioeconómica y etnia autoidentificada.
  • Se evaluaron la calibración y la discriminación y se compararon con los PCE de 2013.

Principales resultados:

  • Las nuevas ecuaciones PREDICT mostraron un rendimiento significativamente mejor que las ECP en todas las métricas.
  • Los PCE sobreestimaron el riesgo de ECV aterosclerótica dura en ~ 40% (hombres) y ~ 60% (mujeres).
  • La privación socioeconómica y el origen étnico fueron predictores independientes, con un mayor riesgo en las poblaciones maoríes, del Pacífico e indias.

Conclusiones:

  • Las ecuaciones de riesgo de ECV específicas de Nueva Zelanda son más relevantes para las poblaciones contemporáneas de atención primaria.
  • Las ecuaciones existentes como las ECP pueden sobreestimar el riesgo en la población general pero subestimarlo en los subgrupos vulnerables.
  • La incorporación de factores socioeconómicos y étnicos es crucial para una evaluación precisa del riesgo de ECV y una atención equitativa.
