Sus antecedentes:

  • El cáncer de próstata afecta al 13% de los hombres en los Estados Unidos, con un riesgo de mortalidad de por vida del 2,5%.
  • Muchos cánceres de próstata son asintomáticos y se detectan mediante exámenes de detección.
  • Los hombres afroamericanos y aquellos con antecedentes familiares enfrentan mayores riesgos.

Objetivo del estudio:

  • Actualizar la recomendación del Grupo de Trabajo de Servicios Preventivos de los Estados Unidos (USPSTF) de 2012 sobre el cribado basado en PSA para el cáncer de próstata.
  • Evaluar los beneficios y los daños del cribado del PSA y el tratamiento posterior.
  • Para analizar el cribado en subpoblaciones de alto riesgo.

Principales métodos:

  • El USPSTF examinó la evidencia sobre los beneficios y los daños del cribado del PSA.
  • Modelos de análisis de decisiones revisados y tasas de sobrediagnóstico.
  • Examinó el cribado en hombres mayores, afroamericanos y aquellos con antecedentes familiares.

Principales resultados:

  • El cribado del PSA en hombres de 55 a 69 años puede prevenir 1,3 muertes por cada 1000 hombres durante 13 años.
  • El cribado puede prevenir 3 casos de metástasis por cada 1000 hombres.
  • Los daños incluyen falsos positivos, sobrediagnóstico, disfunción eréctil e incontinencia; los daños aumentan con la edad.

Conclusiones:

  • Para los hombres de 55 a 69 años, la decisión de realizar un cribado de PSA es individual, equilibrando los pequeños beneficios con los daños como la incontinencia y la disfunción eréctil.
  • Los médicos no deben seleccionar a los hombres que no expresan una preferencia.
  • El cribado del PSA no se recomienda para los hombres mayores de 70 años debido a que los daños son mayores que los beneficios.
