Sus antecedentes:

  • La fiebre reumática aguda (FRA) se deriva de una reacción autoinmune a las infecciones de garganta por Streptococcus pyogenes.
  • La enfermedad cardíaca reumática (RHD), una complicación de la FRA, contribuye a la insuficiencia cardíaca y la muerte prematura.
  • El ARF y el RHD afectan desproporcionadamente a los países de bajos ingresos, exacerbados por la pobreza y la superpoblación.

Objetivo del estudio:

  • Resaltar los importantes desafíos de salud pública planteados por el ARF y el RHD.
  • Identificar las brechas críticas en el diagnóstico, la prevención y el tratamiento de la FRA y la ERH.
  • Hacer hincapié en la necesidad urgente de una investigación de alta calidad para combatir estas enfermedades.

Principales métodos:

  • Este estudio es una revisión de la comprensión actual de ARF y RHD.
  • Sintetiza el conocimiento existente sobre los mecanismos de la enfermedad, la prevalencia y los esfuerzos de control.
  • Identifica las limitaciones de los enfoques diagnósticos y terapéuticos actuales.

Principales resultados:

  • El diagnóstico de ARF es puramente clínico, carece de un estándar de oro de laboratorio.
  • No se ha demostrado que los tratamientos actuales detengan la progresión de ARF a RHD.
  • La evidencia que respalda las estrategias de prevención actuales, como el tratamiento inmediato de la faringitis estreptocócica y los antibióticos a largo plazo, es débil.

Conclusiones:
