Sus antecedentes:

  • La interpretación automatizada de imágenes cardíacas puede mejorar la práctica clínica, en particular para las evaluaciones en serie en la atención primaria.
  • Los avances en la visión por computadora ofrecen el potencial para una línea de análisis de ecocardiograma totalmente automatizada.

Objetivo del estudio:

  • Desarrollar y evaluar una tubería escalable y automatizada para la interpretación de ecocardiogramas utilizando redes neuronales convolucionales.
  • La tubería tiene como objetivo la identificación de vistas, la segmentación de imágenes, la cuantificación de la estructura/función y la detección de enfermedades.

Principales métodos:

  • Modelos de redes neuronales convolucionales entrenados en 14.035 ecocardiogramas para la identificación visual y la segmentación de la cámara cardíaca.
  • Estructura y función cardíacas cuantificadas (fracción de eyección, tensión longitudinal) utilizando la salida de segmentación.
  • Desarrolló modelos para detectar cardiomiopatía hipertrófica, amiloide cardíaca y hipertensión arterial pulmonar.

Principales resultados:

  • Identificación automática precisa de la vista (96% para el eje largo parasternal) y segmentación de la cámara.
  • Las mediciones de la estructura cardíaca mostraron acuerdo con los valores clínicos (por ejemplo, desviación absoluta media del 15 al 17% para los volúmenes).
  • Las mediciones automatizadas de la función (fracción de eyección, deformación) coincidieron con el software comercial y demostraron la utilidad del monitoreo en serie.
  • Los modelos de detección de enfermedades alcanzaron un alto rendimiento (C-estadísticas: 0,93 para la cardiomiopatía hipertrófica, 0,87 para la amiloide cardíaca, 0,85 para la hipertensión arterial pulmonar).

Conclusiones:

  • La tubería de interpretación automática del ecocardiograma proporciona una base para el análisis escalable de los datos de imágenes cardíacas archivados.
  • Esta tecnología admite el seguimiento en serie de pacientes y una amplia aplicación clínica de la interpretación del ecocardiograma.
