Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. Resumen

La monoterapia con upadacitinib mejoró significativamente los resultados de la artritis reumatoide en comparación con la continuación del metotrexato en pacientes con una respuesta inadecuada. Los perfiles de seguridad fueron comparables a los de estudios anteriores, lo que indica una relación beneficio-riesgo favorable para este inhibidor de JAK1.

Sus antecedentes:

  • Upadacitinib es un inhibidor selectivo de Janus kinase (JAK) 1 administrado por vía oral.
  • Estudios anteriores han demostrado su eficacia en el tratamiento combinado de la artritis reumatoide (AR).
  • Este estudio evaluó el upadacitinib como monoterapia en pacientes con AR con respuesta inadecuada al metotrexato.

Objetivo del estudio:

  • Evaluar la seguridad y eficacia de la monoterapia con upadacitinib frente a la continuación del metotrexato en pacientes con AR.
  • Comparar los resultados clínicos y funcionales entre los grupos de tratamiento.

Principales métodos:

  • El ensayo SELECT- MONOTHERAPY incluyó a adultos con AR activa a pesar del tratamiento estable con metotrexato.
  • Los pacientes fueron asignados al azar para recibir upadacitinib (15 mg o 30 mg) o continuar con el metotrexato.
  • Los parámetros de valoración primarios incluyeron la respuesta ACR20 y la baja actividad de la enfermedad (DAS28- CRP ≤ 3,2) a la semana 14.

Principales resultados:

  • Upadacitinib (15 mg y 30 mg) demostró tasas de respuesta ACR20 significativamente más altas (68% y 71%) en comparación con el metotrexato continuado (41%) en la semana 14 (p< 0, 0001).
  • Significativamente más pacientes lograron una baja actividad de la enfermedad con upadacitinib (45% y 53%) frente al metotrexato (19%) (p< 0, 0001).
  • Las tasas de eventos adversos fueron similares entre los grupos; los eventos específicos como el herpes zóster, las neoplasias malignas y los eventos cardiovasculares fueron poco frecuentes.

Conclusiones:

  • La monoterapia con upadacitinib proporciona mejoras estadísticamente significativas en los resultados clínicos y funcionales en pacientes con AR con insuficiencia de metotrexato.
  • El perfil de seguridad de la monoterapia con upadacitinib se alinea con los hallazgos previos en estudios de AR.
  • El cambio a la monoterapia con upadacitinib es una opción de tratamiento viable para esta población de pacientes.
