Sus antecedentes:

  • La guía ACC / AHA BP de 2017 introdujo el riesgo de enfermedad cardiovascular aterosclerótica para iniciar el medicamento antihipertensivo.
  • El papel de la lesión crónica del miocardio y los biomarcadores de estrés en la estratificación del riesgo de presión arterial elevada y hipertensión sigue sin estar claro.

Objetivo del estudio:

  • Investigar si los biomarcadores de la lesión del miocardio (troponina T cardíaca de alta sensibilidad) y el estrés (NT-proBNP) pueden informar la estratificación del riesgo cardiovascular y las decisiones de tratamiento en adultos con presión arterial elevada e hipertensión.

Principales métodos:

  • Datos combinados de 3 estudios de cohorte (ARIC, DHS, MESA), excluyendo la prevalencia de la enfermedad cardiovascular y el uso basal de antihipertensivos.
  • Los participantes estratificados por grupos de tratamiento de la presión arterial y estado del biomarcador (troponina cardíaca de alta sensibilidad ≥6 ng/ L, NT-proBNP ≥100 pg/ ml).
  • Las tasas de incidencia acumuladas estimadas de eventos cardiovasculares y el número de años necesarios para tratar la disminución intensiva de la presión arterial.

Principales resultados:

  • Se incluyeron 12.987 participantes; 825 incidentes cardiovasculares en 10 años.
  • Aumento de la presión arterial/hipertensión sin medicación recomendada: incidencia cardiovascular a 10 años 11,0% con biomarcadores elevados frente al 4,6% sin ellos.
  • Hipertensión en estadio 1 o 2 con medicación recomendada: incidencia cardiovascular a 10 años del 15,1% con biomarcadores elevados frente al 7,9% sin ellos.

Conclusiones:

  • La troponina T cardíaca de alta sensibilidad elevada o NT-proBNP identifica a las personas de alto riesgo con presión arterial elevada o hipertensión que actualmente no se recomienda para la medicación.
  • Se observó un menor riesgo con biomarcadores no elevados, incluso en la hipertensión de estadio 1 o 2.
  • La incorporación de biomarcadores en los algoritmos de riesgo puede optimizar el control intensivo de la presión arterial que coincide con el riesgo del paciente.
