Resumen

La ecocardiografía revela que si bien la función sistólica del ventrículo izquierdo a menudo se conserva en pacientes con COVID-19, la disfunción del ventrículo derecho y la disfunción diastólica son comunes. El deterioro afecta la función de RV más significativamente.

Objetivo del estudio:

  • Evaluar sistemáticamente la función cardíaca en pacientes hospitalizados con COVID-19 utilizando la ecocardiografía.
  • Identificar patologías cardíacas comunes y su asociación con los resultados clínicos.

Principales métodos:

  • 100 pacientes consecutivos con COVID-19 se sometieron a una ecocardiografía completa dentro de las 24 horas posteriores al ingreso.
  • Evaluación de la función ventricular izquierda (LV) y ventricular derecha (RV), hemodinámica de la válvula y ecografía pulmonar.
  • Se realizó una ecocardiografía de seguimiento en casos de deterioro clínico.

Principales resultados:

  • El 32% de los pacientes tenía ecocardiogramas normales al inicio.
  • La dilatación y la disfunción de la RV fueron las más comunes (39%), seguidas de la disfunción de la LV diastólica (16%) y sistólica (10%).
  • La peor función de la RV se asoció con un aumento de la troponina y un peor estado clínico; la disfunción de la RV fue prominente en pacientes con deterioro clínico.

Conclusiones:

  • El COVID-19 con frecuencia afecta la función RV y la función diastólica de LV, mientras que la función sistólica de LV generalmente se conserva.
  • El deterioro clínico a menudo está relacionado con la disfunción aguda de RV, a veces con trombosis venosa profunda.
  • El aumento de la troponina y la gravedad clínica se correlacionan con una peor función RV.
