Sus antecedentes:

  • La enfermedad inflamatoria intestinal (EII) es una afección crónica relacionada con un mayor riesgo de cáncer gastrointestinal.
  • Comprender el panorama genómico de la EII es crucial para dilucidar los mecanismos de la enfermedad y el desarrollo del cáncer.

Objetivo del estudio:

  • Investigar los patrones de mutación somática en las células epiteliales del colon de pacientes con EII.
  • Comparar el panorama de mutación de los colon afectados por EII con el de los colon sanos.
  • Identificar genes y vías bajo selección positiva en EII.

Principales métodos:

  • Secuenciación de todo el genoma de 446 criptas de colon de 46 pacientes con EII.
  • Comparación con 412 criptas colónicas de 41 controles sin EII.
  • Análisis de las tasas de mutación, expansiones clonales y presiones de selección.

Principales resultados:

  • Las células epiteliales del colon con EII presentan una tasa de mutación promedio 2,4 veces mayor que las de los controles sanos.
  • Los procesos mutacionales acelerados, comunes en los colon normales, impulsan este aumento.
  • Se observaron expansiones clonales generalizadas a escala milimétrica en la EII, a diferencia del colon normal.
  • La selección positiva identificó mutaciones no sinónimas en los genes de las vías ARID1A, FBXW7, PIGR, ZC3H12A e IL-17/TLR.

Conclusiones:

  • Los mecanismos de selección distintos operan en el colon afectado por la colitis en comparación con el colon normal.
  • Las mutaciones somáticas pueden desempeñar un papel causal en la patogénesis de la enfermedad inflamatoria intestinal.
  • Estos hallazgos proporcionan información sobre el desarrollo del cáncer asociado a la EII.
