Resumen

Sus antecedentes:

  • La concentración de partículas HDL (HDL-P) puede ofrecer una predicción superior del riesgo de enfermedad cardiovascular aterosclerótica (ASCVD).
  • Existen datos limitados sobre las asociaciones de HDL-C y HDL-P con accidente cerebrovascular isquémico e infarto de miocardio (IM) en mujeres y poblaciones negras.

Objetivo del estudio:

  • Investigar las asociaciones comparativas de HDL-C y HDL-P con el infarto de miocardio y el ictus isquémico.
  • Examinar estas asociaciones específicamente dentro de las poblaciones femeninas y negras.
  • Para probar la hipótesis de que el HDL-P es un predictor más consistente de MI y accidente cerebrovascular que el HDL-C.

Principales métodos:

  • Análisis combinado de los datos de los participantes individuales de cuatro grandes estudios de población (DHS, ARIC, MESA y PREVEND).
  • Inclusión de participantes sin enfermedad cardiovascular aterosclerótica preexistente.
  • Se utilizaron modelos de riesgo proporcional de Cox ajustados para evaluar la relación entre los marcadores de HDL y el infarto de miocardio incidente y el ictus isquémico.

Principales resultados:

  • En la población general, un HDL-P más alto se asoció constantemente con un menor riesgo de infarto de miocardio y accidente cerebrovascular combinados, superando al HDL-C.
  • El ajuste por HDL-P atenuó la asociación de HDL-C con los eventos, mientras que el ajuste por HDL-C no disminuyó la asociación de HDL-P.
  • HDL-P mostró asociaciones inversas consistentes con MI y accidente cerebrovascular en mujeres, pero las asociaciones con MI diferían significativamente por raza en la población general, con una asociación inversa en blancos y una asociación directa en negros.

Conclusiones:

  • El HDL-P demuestra una asociación más consistente con el infarto de miocardio y el riesgo de accidente cerebrovascular isquémico en comparación con el HDL-C en la población general.
  • Las asociaciones diferenciales de los marcadores HDL con el infarto de miocardio en individuos negros sugieren perfiles de riesgo específicos de la etnia.
  • Se necesita más investigación para explorar los resultados cardiovasculares individuales y las variaciones étnicas en la predicción del riesgo.
