Sus antecedentes:

  • El ensayo DAPA- HF investigó la eficacia de dapagliflozina en pacientes con insuficiencia cardíaca (HF).
  • Este análisis se centró en los eventos de empeoramiento de la IH en pacientes ambulatorios y en el impacto de dapagliflozina.

Objetivo del estudio:

  • Evaluar la frecuencia y el pronóstico significativo de los episodios de agravamiento de la IH en pacientes ambulatorios.
  • Evaluar el efecto de la dapagliflozina en estos eventos además de los resultados primarios.

Principales métodos:

  • Se incluyeron pacientes con NYHA clase II-IV, LVEF ≤40% y niveles elevados de NT-proBNP.
  • El estudio analizó un resultado compuesto ampliado que incluía el aumento de la terapia oral para el empeoramiento de la IH.

Principales resultados:

  • La dapagliflozina redujo el resultado compuesto expandido en un 27% (HR 0,73).
  • Los eventos de empeoramiento en pacientes ambulatorios fueron frecuentes (8,6%) y asociados con un mayor riesgo de mortalidad (HR 2,67).
  • Dapagliflozin redujo significativamente todos los componentes del resultado ampliado.

Conclusiones:

  • Los episodios de empeoramiento de la IH en pacientes ambulatorios son frecuentes, son de importancia pronóstica y se reducen con dapagliflozina.
  • La dapagliflozina ofrece beneficios adicionales más allá de prevenir las hospitalizaciones y la muerte cardiovascular.
