Resumen

Los desfibriladores cardioverter implantables (DCI) en pacientes con tetralogía de Fallot muestran altas tasas de tratamiento adecuado, especialmente en la prevención primaria. Sus antecedentes:

  • La tetralogía de Fallot (TOF) es la enfermedad cardíaca congénita cianótica más común.
  • La muerte cardíaca súbita es una causa significativa de mortalidad en pacientes con TOF.
  • Existen datos limitados sobre el uso del desfibrilador cardioverter implantable (DCI) en esta población.

Objetivo del estudio:

  • Evaluar la eficacia y seguridad de los DIC en pacientes con Tetralogía de Fallot.
  • Evaluar las tasas y complicaciones adecuadas del tratamiento con DIC.
  • Identificar los predictores de eventos adversos en pacientes con TOF con DCI.

Principales métodos:

  • En 2010 se estableció un registro nacional francés de pacientes con TOF con ICD.
  • Los eventos clínicos fueron juzgados centralmente por un comité ciego.
  • El seguimiento incluyó el tiempo hasta el primer tratamiento apropiado con DCI, complicaciones, trasplante de corazón y muerte.

Principales resultados:

  • Se incluyeron 165 pacientes con TOF (edad media de 42,2 años, 70,1% varones), el 63% en prevención secundaria.
  • Durante un seguimiento mediano de 6,8 años, el 47,3% recibió un tratamiento adecuado con DIC.
  • Se observaron altas tasas de complicaciones del DCI (43,0%), incluidas las descargas inadecuadas (25,5%) y la disfunción del plomo (21,8%).
  • La fragmentación del QRS predijo de forma independiente las terapias apropiadas del CDI (HR, 3.47).
  • Insuficiencia cardíaca congestiva o reducción del FEV aumentó el riesgo de muerte no arritmica o trasplante cardíaco (HR, 11.01).

Conclusiones:

  • Los pacientes con TOF con ICD experimentan altas tasas de terapias apropiadas, incluso en la prevención primaria.
  • La carga sustancial de complicaciones de la CDI requiere una selección cuidadosa de los pacientes.
  • La integración de la fragmentación del QRS en los modelos de estratificación de riesgo puede mejorar la selección de pacientes para la implantación del DIC.
