Sus antecedentes:

  • El desarrollo de vacunas efectivas contra el SARS-CoV-2 se ha visto obstaculizado por la falta de correlatos de inmunidad claramente definidos.
  • Si bien los anticuerpos neutralizantes están asociados con la protección contra otros virus, su suficiencia para la protección del SARS-CoV-2 en la población general es incierta.

Objetivo del estudio:

  • Investigar la evolución temprana de la respuesta inmune humoral en pacientes hospitalizados con infección moderada a grave por SARS-CoV-2.
  • Identificar distintas trayectorias inmunes humoral asociadas con la gravedad y los resultados de la enfermedad.

Principales métodos:

  • Análisis de las respuestas inmunes humoral, incluidos los niveles de anticuerpos IgM, IgA e IgG.
  • Evaluación de la unión al receptor gamma Fc y la función del efector Fc en 193 individuos hospitalizados.
  • Comparación de las respuestas inmunes entre los supervivientes y los no supervivientes, y entre los grupos de enfermedad moderada y grave.

Principales resultados:

  • Los pacientes con enfermedad grave que no sobrevivieron presentaron respuestas de IgG atenuadas, compromiso de la unión al receptor gamma Fc y reducción de la actividad del efector Fc.
  • Se observaron respuestas robustas de IgM e IgA tanto en los supervivientes como en los no supervivientes con enfermedad grave.
  • Las personas con enfermedad moderada mostraron respuestas humoral tardías pero finalmente maduras.

Conclusiones:

  • El desarrollo humoral deficiente, en lugar de la inmunidad que mejora la enfermedad, caracterizó a los no sobrevivientes de la infección severa por SARS-CoV-2.
  • Se asocian distintas trayectorias inmunológicas humoral con la resolución de la infección por SARS-CoV-2.
  • El desarrollo temprano de inmunidad humoral funcional es esencial para el control efectivo de la infección por SARS-CoV-2.
