Resumen

Sus antecedentes:

  • El síndrome inflamatorio multisistémico en niños (MIS-C) es una afección grave y potencialmente mortal relacionada con la infección por SARS-CoV-2.
  • La estrategia terapéutica inicial óptima para el MIS-C sigue siendo indeterminada.

Objetivo del estudio:

  • Comparar la eficacia del tratamiento inicial con inmunoglobulinas intravenosas (IVIG) más metilprednisolona frente al IVIG solo en niños diagnosticados con MIS-C.
  • Para evaluar las tasas de fracaso del tratamiento, la necesidad de terapias de segunda línea, el apoyo hemodinámico, la disfunción cardíaca y la duración de la estancia en la unidad de cuidados intensivos pediátricos (UCIP).

Principales métodos:

  • Un estudio de cohorte retrospectivo que utiliza datos de un sistema de vigilancia nacional francés.
  • Se utilizó el análisis de propensión por puntuación para comparar los resultados entre los niños que recibieron IVIG más metilprednisolona y los que recibieron solo IVIG.
  • Se incluyeron casos confirmados de MIS-C que cumplen con los criterios de la Organización Mundial de la Salud, con datos de seguimiento recopilados entre abril de 2020 y enero de 2021.

Principales resultados:

  • El tratamiento con IVIG y metilprednisolona se asoció con un riesgo significativamente menor de fracaso del tratamiento en comparación con IVIG solo (9% frente a 51%).
  • La terapia combinada también demostró reducciones significativas en la necesidad de terapia de segunda línea, apoyo hemodinámico y disfunción ventricular izquierda aguda.
  • Los niños tratados con IVIG y metilprednisolona tuvieron una estancia mediana más corta en la UIPC (4 días frente a 6 días).

Conclusiones:

  • El tratamiento inicial de MIS-C con IVIG más metilprednisolona parece ser más eficaz que el IVIG solo, lo que lleva a un curso de la fiebre más favorable.
  • Aunque es prometedor, la naturaleza observacional de este estudio requiere una interpretación cautelosa de los hallazgos.
