Sus antecedentes:

  • La medición de la presión arterial en el consultorio es el método convencional para el diagnóstico de la hipertensión, pero carece de precisión.
  • El monitoreo de la presión arterial fuera del consultorio, incluido el monitoreo de la presión arterial en el hogar (HBPM) y el monitoreo ambulatorio de la presión arterial durante 24 horas (ABPM), ofrece alternativas.

Objetivo del estudio:

  • Revisar sistemáticamente la precisión de los métodos de medición de la presión arterial en las oficinas y en el hogar.
  • Evaluar la capacidad de estos métodos para clasificar correctamente a los adultos con hipertensión.
  • La clasificación de la hipertensión se definió utilizando el ABPM de 24 horas como estándar de referencia.

Principales métodos:

  • Se realizó una búsqueda exhaustiva de la literatura en varias bases de datos (PubMed, Cochrane Library, Embase, etc.). hasta abril de 2021.
  • Dos autores abstrajeron los datos de forma independiente y evaluaron la calidad del estudio; un tercer autor resolvió las discrepancias.
  • Se utilizaron modelos de efectos aleatorios para calcular los ratios de sensibilidad, especificidad y probabilidad sumarios (LR) para los métodos de medición de la presión arterial.

Principales resultados:

  • Doce estudios (n=6877) evaluaron las mediciones de la presión arterial en el consultorio, y seis estudios (n=2049) evaluaron la HBPM en comparación con la ABPM de 24 horas.
  • Las mediciones convencionales de presión arterial en consultorios (≥ 140/ 90 mm Hg) mostraron una sensibilidad del 51% y una especificidad del 88%.
  • El monitoreo de la presión arterial en el hogar (HBPM) (≥ 135/85 mm Hg) demostró una mayor sensibilidad (75%) y especificidad (76%) en comparación con las mediciones en el consultorio.

Conclusiones:

  • Las mediciones de la presión arterial en el consultorio pueden no ser lo suficientemente precisas para diagnosticar o excluir definitivamente la hipertensión.
  • El monitoreo de la presión arterial en el hogar puede ser una herramienta valiosa para confirmar un diagnóstico de hipertensión.
  • Cuando las lecturas de la presión arterial en el consultorio y en el hogar son discordantes o inciertas, se recomienda un ABPM de 24 horas para el diagnóstico definitivo.
