Sus antecedentes:

  • La duración óptima de la terapia antiplaquetaria (TPA) en pacientes con alto riesgo de hemorragia después de la colocación de un stent coronario es incierta.
  • El análisis de subgrupos del ensayo MASTER DAPT investigó los resultados basados en la indicación de anticoagulación oral (OAC).

Objetivo del estudio:

  • Evaluar la eficacia y seguridad de los regímenes abreviados frente a los regímenes estándar de APT en pacientes con alto riesgo de hemorragia.
  • Para comparar los resultados en pacientes con y sin una indicación de OAC después de la colocación de un stent coronario.

Principales métodos:

  • El ensayo MASTER DAPT aleatorizado y abierto en el que participaron 4579 pacientes después de la colocación de un stent.
  • Los pacientes recibieron APT dual de 1 mes seguido de estrategias de APT abreviadas o no abreviadas, estratificadas por indicación OAC.
  • Los resultados coprimarios incluyeron resultados clínicos adversos netos, eventos cardíacos y cerebrales adversos importantes y eventos de sangrado del Consorcio de Investigación Académica (BARC).

Principales resultados:

  • No hubo diferencias significativas en los resultados clínicos adversos netos o en los acontecimientos adversos cardíacos y cerebrales importantes entre la TPA abreviada y la TPA estándar, independientemente de la indicación OAC.
  • La APT abreviada mostró una tendencia hacia la reducción de hemorragias BARC 2, 3 o 5 en pacientes sin indicación de OAC (HR, 0,55; Pinteracción=0, 057).
  • Se observó una reducción significativa de la hemorragia BARC 2 con APT abreviada en pacientes sin indicación de OAC (HR, 0,48; Pinteracción=0,021).

Conclusiones:

  • Los regímenes abreviados de APT no son inferiores a los regímenes estándar en cuanto a los principales resultados clínicos adversos y los resultados clínicos adversos netos en pacientes con alto riesgo de hemorragia.
  • La duración más corta de la APT condujo a tasas de sangrado significativamente más bajas en pacientes sin una indicación de OAC.
  • Estos hallazgos apoyan una duración más corta de la APT para pacientes seleccionados con alto riesgo de hemorragia después de la colocación de un stent.
