Resumen

El uso persistente de opiáceos (POU) es común después de procedimientos de dispositivo electrónico implantable cardíaco (CIED, por sus siglas en inglés), afectando al 12% de los pacientes. Las dosis iniciales más altas de opioides aumentan el riesgo de desarrollar UPO.

Sus antecedentes:

  • Se desconoce la incidencia de la UPO en pacientes que no han ingerido opioides previamente después de procedimientos con dispositivos electrónicos implantables cardíacos (DIEC).

Objetivo del estudio:

  • Determinar la incidencia de la UPO entre los pacientes que no han ingerido opiáceos antes después de los procedimientos CIED.
  • Identificar los factores de riesgo asociados con la UPO en esta población de pacientes.

Principales métodos:

  • Estudio de cohorte retrospectivo basado en una base de datos nacional de reclamaciones administrativas (2004-2018).
  • Incluidos los pacientes adultos que no han ingerido opioides y que se someten a procedimientos de CIED, excluidos los que se someten a otros procedimientos dentro de los 180 días.
  • Se define POU como la prescripción de opiáceos más de 30 días después del procedimiento CIED.

Principales resultados:

  • El 11% de los 143.400 pacientes elegibles recibieron opiáceos dentro de los 14 días de la cirugía CIED.
  • La UPO se produjo en el 12,4% de estos pacientes (30-180 días después de la cirugía).
  • Los factores de riesgo para la UPO incluyen el abuso previo de drogas, el uso de relajantes musculares/benzodiazepinas y dosis iniciales más altas de morfina oral equivalente (> 135 mg).

Conclusiones:

  • La UPO es un resultado común después de los procedimientos CIED, con el 12% de los pacientes que continúan con el uso de opioides más allá de los 30 días.
  • Las dosis iniciales más altas de opiáceos recetados están relacionadas con un mayor riesgo de desarrollar UPO.
