Sus antecedentes:

  • La fibrilación auricular (FA) es la arritmia cardíaca más común, que aumenta con la edad y representa un riesgo significativo de accidente cerebrovascular isquémico.
  • Alrededor del 20% de los accidentes cerebrovasculares relacionados con la FA conducen a un diagnóstico inicial de FA.
  • La prevalencia de AF varía según la edad y el sexo, pero las diferencias entre la raza y el origen étnico siguen siendo inciertas.

Objetivo del estudio:

  • Para actualizar las recomendaciones de 2018, la USPSTF revisó sistemáticamente los beneficios y daños de la detección de AF en adultos mayores.
  • La revisión evaluó la precisión de las pruebas de cribado, la eficacia en la detección de FA no diagnosticada y el impacto de la terapia anticoagulante en los casos detectados por cribado.

Principales métodos:

  • Revisión sistemática encargada por el Grupo de Trabajo de Servicios Preventivos de los Estados Unidos (USPSTF).
  • Centrado en adultos asintomáticos de 50 años o más sin antecedentes de ataque isquémico transitorio o accidente cerebrovascular.
  • Pruebas de detección evaluadas, eficacia de la detección y beneficios/daños del tratamiento.

Principales resultados:

  • No existen pruebas sobre los beneficios y los daños del cribado de la fibrilación auricular en adultos asintomáticos.
  • No se pudo determinar el equilibrio entre los beneficios y los daños del cribado de AF en esta población.

Conclusiones:

  • El USPSTF concluyó que las pruebas actuales son insuficientes para evaluar el equilibrio de beneficios y daños del cribado de la FA.
  • Se emitió una declaración "I", indicando evidencia insuficiente a favor o en contra del cribado.
