Sus antecedentes:
- La candidiasis sistémica es una infección frecuente y letal adquirida en el hospital.
- Es una complicación emergente de la terapia de anticuerpos monoclonales C5 anticomplemento.
- Esto pone de relieve el papel crucial de C5 en la inmunidad antifúngica.
Objetivo del estudio:
- Investigar el papel del sistema del complemento en la candidiasis sistémica.
- Para identificar los biomarcadores predictivos de la candidemia.
- Elucidar los mecanismos de la protección antifúngica mediada por C5.
Principales métodos:
- Análisis de la expresión génica en pacientes candidémicos.
- Utilizando un modelo de ratón de la candidiasis sistémica.
- Investigar el impacto de la ablación del receptor C5a 1 (C5ar1) en la respuesta del huésped y la supervivencia.
Principales resultados:
- La transcripción génica del sistema de complemento fue la vía inducida superior en pacientes con candidemia y candidemia predicha.
- La señalización C5a-C5aR1 promovió el aclaramiento fúngico y la supervivencia del huésped a través de la función fagocítica y las vías de supervivencia tisular (ERK, AKT).
- La deficiencia de C5ar1 alteró el metabolismo de los macrófagos, indujo la apoptosis y aumentó la mortalidad debido a la lesión renal.
Conclusiones:
- El C5 local derivado de los fagocitos es esencial para una función antimicrobiana óptima de los fagocitos.
- La producción de C5 por los fagocitos confiere una protección innata contra las infecciones fúngicas sistémicas.
- Los niveles reducidos de C5a o los polimorfismos de C5 que afectan la expresión de leucocitos C5 se correlacionan con resultados adversos en los pacientes.
