Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

Sus antecedentes:

  • La persistencia a largo plazo de los beneficios de los inhibidores del cotransportador de sodio-glucosa 2 (SGLT2i) en el tratamiento de la insuficiencia cardíaca sigue sin estar clara.
  • En ensayos previos como EMPEROR-Reducido y EMPEROR-Preservado se demostró la eficacia durante una mediana de 16 a 26 meses.

Objetivo del estudio:

  • Investigar si los beneficios cardiovasculares y sintomáticos de la empagliflozina en la insuficiencia cardíaca persisten después de la interrupción del tratamiento.
  • Evaluar el impacto de la interrupción del tratamiento con empagliflozina en los resultados clínicos y los marcadores fisiológicos.

Principales métodos:

  • Los pacientes de los ensayos EMPEROR-Reducido y EMPEROR-Preservado fueron retirados del tratamiento con empagliflozina o placebo.
  • Un subconjunto de pacientes se sometió a evaluaciones presenciales aproximadamente 30 días después de la retirada del medicamento.
  • Se evaluaron los resultados clínicos (muerte cardiovascular, hospitalización por insuficiencia cardíaca) y los síntomas informados por los pacientes (cuestionario de cardiomiopatía de Kansas City).

  • Los síntomas informados por los pacientes, medidos por el Cuestionario de Cardiomiopatía de Kansas City, disminuyeron después de la retirada de empagliflozina.
  • La suspensión de empagliflozina revirtió los cambios fisiológicos beneficiosos, incluyendo mejoras en los niveles de glucosa, peso corporal y péptidos natriuréticos.

Conclusiones:

  • La empagliflozina demuestra un efecto beneficioso persistente en pacientes con insuficiencia cardíaca, incluso después de un tratamiento prolongado.
