Resumen
El aborto inducido en el primer trimestre no representa un riesgo de sensibilización Rh. La prueba de Rh y el tratamiento con inmunoglobulina Rh generalmente no son necesarios antes de las 12 semanas de gestación, lo que simplifica el cuidado.

Objetivo del estudio:

- Evaluar la necesidad de la prueba de Rh y la administración de inmunoglobulina Rh en la interrupción temprana del embarazo.
Principales métodos:
- Se realizó un estudio de cohorte multicéntrico, observacional y prospectivo.
- La citometría de flujo de alto rendimiento analizó muestras de sangre materna emparejadas antes y después del aborto inducido del primer trimestre (medicamento o procedimiento) en 506 participantes.
- El resultado primario medido fue la proporción de participantes con recuentos de glóbulos rojos por encima del umbral de sensibilización (125 glóbulos rojos/5 millones de glóbulos rojos).
Principales resultados:
- Sólo el 0,2% de las participantes tenían recuentos elevados de glóbulos rojos por encima del umbral de sensibilización después del aborto.
- No se encontró una asociación significativa entre las características basales y el recuento de glóbulos rojos después del aborto.
- El cambio mediano en el recuento de glóbulos rojos desde el inicio fue cero, con aumentos mínimos observados.
Conclusiones:
- El aborto inducido en el primer trimestre no es un factor de riesgo para la sensibilización Rh.
- La prueba de Rh y el tratamiento con inmunoglobulina Rh probablemente no sean necesarios antes de las 12 semanas de gestación.
- Los hallazgos pueden servir de base para las directrices internacionales sobre el uso de inmunoglobulina Rh después de la interrupción temprana del embarazo.
