Resumen
La inhibición del reclutamiento de células mieloides revirtió la resistencia al tratamiento en el cáncer de próstata. La quimiotaxis mieloide dirigida redujo la inflamación tumoral y mejoró los resultados en pacientes con cáncer de próstata metastásico resistente a la castración.

Sus antecedentes:
- La inflamación, particularmente la expansión mieloide indicada por una alta proporción de neutrófilos a linfocitos, está relacionada con una baja supervivencia y resistencia al tratamiento en varios tipos de cáncer.
- El papel de la inflamación mieloide en la conducción de la progresión del cáncer de próstata y la resistencia al tratamiento en humanos no quedó claro.
Objetivo del estudio:
- Investigar si la inhibición de la quimiotaxis mieloide puede reducir la inflamación mieloide provocada por el tumor y superar la resistencia a la terapia en el cáncer de próstata metastásico resistente a la castración (CRPC).
- Determinar si la orientación de los ligandos CXCR2 puede revertir la resistencia a los inhibidores de la señalización de los receptores de andrógenos (ISRA) en CRPC.
Principales métodos:
- Se realizó un ensayo clínico de prueba de concepto que combinaba un inhibidor de CXCR2 (AZD5069) con enzalutamida en pacientes con CRPC metastásico resistente al ARSI.
- Se evaluó el impacto de la terapia combinada en los niveles de neutrófilos circulantes, la infiltración de células mieloides intratumorales y los resultados clínicos (respuestas bioquímicas y radiológicas).
- Relación entre neutrófilos y linfocitos en sangre con infiltración mieloide tumoral y expresión de ARNm asociado a la senescencia, incluidos los ligandos CXCR2.
Principales resultados:
- La terapia combinada fue bien tolerada sin toxicidad limitante de la dosis.
- El tratamiento redujo los neutrófilos circulantes, redujo la infiltración de células mieloides intratumorales (CD11b+HLA-DRloCD15+CD14-), y demostró un beneficio clínico duradero en un subconjunto de pacientes.
- Respuestas bioquímicas y radiológicas observadas en pacientes tratados con inhibidores de CXCR2 y enzalutamida.
Conclusiones:
- La inflamación mieloide asociada a la senescencia puede impulsar la progresión del CRPC metastásico y la resistencia al bloqueo del receptor de andrógenos.
- La inhibición de la quimiotaxis mieloide a través del bloqueo de CXCR2 ofrece una estrategia prometedora para revertir la resistencia a la terapia en un subconjunto de pacientes con CRPC.
- La quimiotaxis mieloide dirigida justifica una mayor investigación en otros tipos de cáncer.
