Sus antecedentes:

  • La enfermedad pulmonar intersticial (ILD) es un grupo de trastornos pulmonares que causan inflamación y fibrosis, lo que lleva a una disnea progresiva y insuficiencia respiratoria.
  • La ILD afecta a aproximadamente 650.000 personas en los Estados Unidos, lo que resulta en 25.000 a 30.000 muertes al año.
  • Los tipos comunes de ILD incluyen fibrosis pulmonar idiopática (IPF), neumonitis por hipersensibilidad y ILD asociada a enfermedades del tejido conectivo.

Objetivo del estudio:

  • Para resumir la comprensión actual y los enfoques de tratamiento para la enfermedad pulmonar intersticial (ILD).
  • Para resaltar el impacto de la ILD en la salud y la supervivencia del paciente.
  • Revisar los métodos de diagnóstico y las intervenciones terapéuticas para varios subtipos de ILD.

Principales métodos:

Principales resultados:

  • La ILD se presenta con disnea por esfuerzo, con tos reportada en el 30% de los pacientes.
  • La TC torácica muestra una alta sensibilidad (91%) y especificidad (71%) para el diagnóstico de subtipos de ILD como IPF.
  • Una disminución del 5% en FVC durante 12 meses duplica el riesgo de mortalidad.
  • Las terapias antifibróticas (nintedanib, pirfenidona) retrasan la disminución de la CVF en un 44-57% en la FPI y la ILD fibrótica progresiva.
  • Las terapias inmunomoduladoras pueden estabilizar o mejorar la CVF en la ILD asociada a la DTC.
  • El ejercicio estructurado y la oxigenoterapia mejoran los síntomas y la calidad de vida.
  • El trasplante de pulmón ofrece una mejor supervivencia (5,2-6,7 años) en comparación con la ausencia de trasplante (< 2 años) para la ILD en etapa terminal.
  • El treprostinil inhalado mejora los síntomas en pacientes con EII con hipertensión pulmonar.

Conclusiones:
