Sus antecedentes:

  • Los anticoagulantes orales directos (DOAC) se utilizan para la prevención del tromboembolismo en la fibrilación auricular.
  • La seguridad y eficacia de los DOAC en los sobrevivientes de hemorragias intracerebrales no están bien establecidas.
  • Existe incertidumbre con respecto al equilibrio entre la reducción del ictus isquémico y el aumento del riesgo de hemorragia recurrente.

Objetivo del estudio:

  • Determinar si los DOAC reducen el riesgo de ictus isquémico en pacientes con fibrilación auricular y antecedentes de hemorragia intracerebral.
  • Evaluar si los DOAC aumentan significativamente el riesgo de hemorragia intracerebral recurrente en este grupo de pacientes.

Principales métodos:

  • PRESTIGE-AF, un ensayo aleatorizado multicéntrico de fase 3.
  • 75 hospitales en seis países europeos.
  • Pacientes mayores de 18 años con hemorragia intracerebral espontánea, fibrilación auricular e indicación para anticoagulación (escala de Rankin modificada ≤4).
  • Randomizado 1:1 a DOAC o sin anticoagulación, estratificado por la ubicación de la hemorragia y el sexo.
  • Endpoints coprimarios: primer ictus isquémico y primera hemorragia intracerebral recurrente.

Principales resultados:

  • Se inscribieron 319 participantes; 158 fueron asignados al azar a DOAC, 161 a ninguna anticoagulación.
  • Mediana de seguimiento: 1,4 años.
  • El grupo DOAC mostró tasas de ictus isquémico significativamente más bajas (HR 0,05, p< 0, 0001).
  • El grupo DOAC no cumplió con la no inferioridad para la hemorragia intracerebral recurrente (HR 10.89).
  • Se observaron tasas más altas de hemorragia intracerebral recurrente en el grupo DOAC (5,00 frente a 0,82 por cada 100 pacientes-años).

Conclusiones:

  • Los DOAC previenen eficazmente los accidentes cerebrovasculares isquémicos en pacientes con fibrilación auricular después de una hemorragia intracerebral.
  • Este beneficio es parcialmente compensado por un riesgo sustancialmente mayor de hemorragia intracerebral recurrente.
  • Se necesitan más investigaciones, incluidos ensayos y metanálisis en curso, para evaluar alternativas más seguras para la prevención del accidente cerebrovascular en esta población vulnerable.
