Los modelos de aprendizaje automático muestran potencial para automatizar las recomendaciones de tratamiento del cáncer de próstata de las juntas multidisciplinarias de tumores (MTB). Si bien es preciso para las terapias locales, se necesita una optimización adicional para un uso clínico más amplio.

Sus antecedentes:

  • Las juntas multidisciplinarias de tumores (MTB) son cruciales para el tratamiento del cáncer de próstata, pero requieren mucho tiempo.
  • La automatización de las recomendaciones del MTB puede mejorar la accesibilidad y la eficiencia.

Objetivo del estudio:

  • Evaluar algoritmos de aprendizaje automático (ML) para automatizar las recomendaciones diagnósticas y terapéuticas en el tratamiento de pacientes con cáncer de próstata.
  • Evaluar el rendimiento de los algoritmos de árbol de decisión, bosque aleatorio y vecinos más cercanos (KNN) en la predicción de decisiones de MTB.

Principales métodos:

  • Se utilizó un conjunto de datos retrospectivo de las recomendaciones MTB de 1929 (2020-2024).
  • Se entrenaron tres algoritmos ML para predecir recomendaciones para PSMA-PET, imágenes convencionales, vigilancia activa y terapia local.
  • El rendimiento del modelo se evaluó utilizando la precisión, la precisión, el recuerdo y la puntuación F1.

Principales resultados:

  • El modelo de Bosque Aleatorio logró la mayor precisión general (66,3%).
  • Se observó una alta precisión para las predicciones de la terapia local (puntuación F1: 0,99).
  • El rendimiento fue menor para las recomendaciones menos frecuentes (PSMA-PET, vigilancia activa) debido al desequilibrio de clase y los cambios en las directrices.

Conclusiones:

  • El ML es prometedor para replicar los patrones de toma de decisiones MTB en el cáncer de próstata.
  • Los modelos actuales requieren una optimización adicional para su aplicación clínica, sirviendo como prueba de concepto.
  • La investigación futura debe centrarse en los datos multiinstitucionales, la validación prospectiva y la adaptación a las directrices en evolución.
