Sus antecedentes:

  • La enfermedad pulmonar intersticial (ILD) afecta a más del 60% de los pacientes con esclerosis sistémica (SSc), lo que lleva a una mortalidad significativa.
  • Los macrófagos alveolares derivados de monocitos (MoAM) están implicados en la patogénesis de la fibrosis pulmonar, requiriendo la señalización del Factor de Estimulación de Colonia 1 (CSF1R) para la persistencia.

Objetivo del estudio:

  • Investigar si la proporción de MoAM en el líquido de lavado broncoalveolar (BAL) puede servir como biomarcador para la gravedad de la SSc-ILD.
  • Explorar el papel del MoAM y los macrófagos intersticiales en la patogénesis de la SSc-ILD y su respuesta al tratamiento.

Principales métodos:

  • Análisis de fluido BAL de 9 pacientes con SSc-ILD y 13 controles sanos utilizando citometría de flujo y secuenciación de ARN de una sola célula.
  • Análisis transcriptómico del MoAM y transcriptómica espacial en explantes pulmonares del SSc-ILD.

Principales resultados:

  • En comparación con los controles, los pacientes con SSc-ILD mostraron un aumento de la MOAM y de los macrófagos intersticiales en el líquido BAL.
  • La abundancia de macrófagos se correlacionó con la gravedad de la fibrosis pulmonar.
  • Se observaron cambios transcriptómicos en el MoAM después del tratamiento con micofenolato, una terapia eficaz de SSc-ILD.

Conclusiones:

  • La proporción de MoAM profibrótico y macrófagos intersticiales en el líquido BAL puede ser un biomarcador valioso para la SSc-ILD.
  • Estas poblaciones de macrófagos representan objetivos terapéuticos potenciales para SSc-ILD.
