Sus antecedentes:

  • La enteroscopia de un solo globo (SBE) es una herramienta clave para diagnosticar y tratar afecciones del intestino delgado, particularmente sangrado gastrointestinal oscuro y anemia por deficiencia de hierro.
  • Sin embargo, la seguridad y la eficacia de SBE en la población de edad avanzada siguen siendo insuficientemente estudiadas.

Objetivo del estudio:

  • Evaluar la seguridad y viabilidad tanto de la EBS antegrada como de la retrógrada en pacientes ancianos.
  • Para comparar el rendimiento diagnóstico, el rendimiento terapéutico y las tasas de complicaciones en diferentes grupos de edad.

Principales métodos:

  • Un estudio de cohorte retrospectivo analizó 284 procedimientos de SBE realizados entre marzo de 2011 y mayo de 2020.
  • Los pacientes se clasificaron en tres grupos: menores de 65, 65-75, y mayores de 75 años.
  • Los análisis estadísticos incluyeron ANOVA, pruebas chi-cuadrado y regresión logística para comparar los resultados.

Principales resultados:

  • Los pacientes de edad avanzada (mayores de 75 años) mostraron una mayor probabilidad de hemorragia gastrointestinal como indicación para el SBE.
  • El rendimiento diagnóstico (68,0%) y las tasas de intervención terapéutica (58,5%) fueron significativamente mayores en el grupo de ancianos.
  • Las angioectasias fueron el hallazgo más común y fueron más frecuentes en los ancianos (44%).
  • Las tasas generales de complicaciones fueron bajas (0,9%) y comparables en todos los grupos de edad.

Conclusiones:

  • La enteroscopia de un solo globo es un procedimiento seguro y factible para pacientes ancianos.
  • SBE demuestra rendimientos diagnósticos y terapéuticos superiores en personas mayores, principalmente debido a una mayor incidencia de angioectasias del intestino delgado.
