Resumen
El parto prematuro ocurre en el 22% de los bebés con trisomía 21, el doble de la tasa general. La edad materna y la falta de atención prenatal aumentan significativamente este riesgo en los bebés con síndrome de Down.

Objetivo del estudio:
- Para identificar los factores maternos y infantiles asociados con el parto prematuro en bebés solteros con trisomía 21.
- Informar sobre posibles intervenciones dirigidas a reducir las tasas de nacimientos prematuros en niños con síndrome de Down.
Principales métodos:
- Estudio epidemiológico descriptivo utilizando el Registro de Defectos de Nacimiento de Texas (1999-2018).

- Análisis de regresión logística multivariable para evaluar las asociaciones entre el parto prematuro y 14 características maternas/infantiles.
Principales resultados:
- Se encontraron asociaciones significativas entre el nacimiento prematuro y la raza/etnia materna, la edad materna (por ejemplo, ≥40 años), el lugar de nacimiento maternal, el acceso a la atención prenatal, el tabaquismo, el sexo del bebé y el año del parto.
- Aumento del riesgo de parto prematuro observado con edad materna avanzada (aOR 1,44) y ausencia de atención prenatal (aOR 1,59).
Conclusiones:
- Los hallazgos mejoran la comprensión del perfil de riesgo de nacimiento prematuro para los bebés con trisomía 21.
- Los factores identificados pueden orientar la investigación futura y el desarrollo de intervenciones específicas para reducir las tasas de nacimientos prematuros en este grupo vulnerable.
