Resumen

La lesión de la arteria pulmonar por un catéter de la arteria pulmonar (PAC) puede causar sangrado masivo durante la cirugía cardíaca. La embolización transcatéter a través del tronco de la arteria pulmonar principal detuvo con éxito el sangrado en dos pacientes, lo que demuestra su valor diagnóstico y terapéutico.

Sus antecedentes:

  • El uso del catéter de la arteria pulmonar (PAC) es común en la cirugía cardíaca.
  • Los PAC pueden conducir a complicaciones raras pero graves, incluida la lesión de la arteria pulmonar.
  • La hemoptisis masiva durante la cirugía cardíaca requiere un diagnóstico y una intervención inmediatos.

Objetivo del estudio:

  • Para reportar dos casos de lesión de la arteria pulmonar intraoperatoria secundaria a la colocación de la PAC.
  • Evaluar la eficacia de la embolización transcatéter para el tratamiento de esta complicación.
  • Para resaltar la utilidad de un enfoque a través del tronco de la arteria pulmonar principal para el diagnóstico y el tratamiento.

Principales métodos:

  • Dos pacientes sometidos a una cirugía cardíaca compleja experimentaron una hemoptisis masiva repentina.
  • Se confirmó el diagnóstico de lesión de la arteria pulmonar debido a la PAC.
  • La arteriografía pulmonar se realizó a través del tronco de la arteria pulmonar principal.
  • Se utilizó la embolización transcatéter para la hemostasis.

Principales resultados:

  • Se logró una hemostasis exitosa en ambos casos mediante la embolización transcatéter.
  • El enfoque a través del tronco de la arteria pulmonar principal facilitó el diagnóstico y el tratamiento.
  • Ambos pacientes se recuperaron después de la intervención.

Conclusiones:

  • La lesión intraoperatoria de la arteria pulmonar por los PAC es una emergencia quirúrgica crítica.
  • La embolización transcatéter a través del tronco de la arteria pulmonar principal es un tratamiento eficaz.
  • Este enfoque ofrece una valiosa opción diagnóstica y terapéutica para el manejo de las lesiones de las arterias pulmonares inducidas por PAC.
