Sus antecedentes:

  • La oclusión de la arteria central de la retina (OCR) está relacionada con la isquemia cerebrovascular.
  • La declaración de la Asociación Americana del Corazón (AHA) de 2021 recomienda evaluaciones de accidente cerebrovascular para pacientes con CRAO.
  • Las tasas anteriores de evaluación de ACV para CRAO antes de 2021 no se comprenden bien.

Objetivo del estudio:

  • Evaluar las tasas nacionales de evaluación del accidente cerebrovascular en pacientes diagnosticados con CRAO antes de las directrices de la AHA de 2021.
  • Identificar patrones y predictores de las evaluaciones del ictus en pacientes con CRAO.

Principales métodos:

  • Análisis retrospectivo de las admisiones de pacientes internados (2016-2021) a partir de la muestra nacional de pacientes internados.
  • Las tasas de finalización medidas de los ecocardiogramas, las imágenes de los vasos de la cabeza y el cuello y las biopsias de la arteria temporal para las admisiones CRAO.
  • Se utilizó el análisis de agrupación para caracterizar los grupos de pacientes y sus patrones de evaluación.

Principales resultados:

  • De las 9615 admisiones CRAO, solo el 0,3% tuvo ecografías carotídicas, el 10% tuvo ecocardiogramas y el 8,5% tuvo imágenes cerebrovasculares.
  • Los ecocardiogramas (10%) y las biopsias de la arteria temporal (6,4%) fueron las evaluaciones más frecuentes.
  • El agrupamiento reveló grupos distintos: mujeres mayores (a menudo en Medicare) que recibieron biopsias, y hombres más jóvenes que recibieron ecocardiogramas / imágenes y fueron dados de alta.
  • El ingreso más alto, el sexo femenino, el estado de transferencia, el accidente cerebrovascular simultáneo, la aterosclerosis carotídea y la dependencia de la nicotina predijeron mayores tasas de evaluación.

Conclusiones:

  • Las tasas nacionales de evaluación de accidentes cerebrovasculares para pacientes con CRAO eran bajas antes de las pautas de la AHA de 2021.
  • Los pacientes con un estatus socioeconómico más alto tenían más probabilidades de recibir evaluaciones completas del accidente cerebrovascular.
  • Los hallazgos ponen de relieve las disparidades en la prevención secundaria del accidente cerebrovascular para el CRAO.
