Sus antecedentes:

  • Las infecciones sospechosas se presentan de manera heterogénea, con un entendimiento limitado de las variaciones de respuesta del huésped y sus implicaciones clínicas.
  • La caracterización de las respuestas del huésped es crucial para mejorar los resultados de los pacientes en entornos de atención aguda.

Objetivo del estudio:

  • Para identificar grupos de respuesta biológicamente discretos en pacientes con sospecha de infecciones que requieren hospitalización.
  • Analizar los correlatos clínicos y los resultados asociados con los grupos de respuesta del huésped identificados.

Principales métodos:

  • Se utilizaron aproximaciones y proyecciones uniformes de variedad (UMAP) y agrupación de medios K en los datos de proteínas plasmáticas de 3802 pacientes.
  • Se aplicaron técnicas de agrupación a 29 proteínas plasmáticas para definir los perfiles de respuesta del huésped.

Principales resultados:

  • Se identificaron dos grupos principales: "desregulados" (proteínas anormales, resultados adversos) y "no diferenciados".
  • Además, se sub-agrupó el grupo "Dysregulated" en cuatro perfiles distintos (agrupaciones 3-6) con firmas biológicas, características clínicas y resultados únicos.
  • Grupos específicos correlacionados con insuficiencia renal (Grupo 3), infecciones virales (Grupo 4), inmunosupresión y malos resultados (Grupo 5) e infecciones bacterianas gramnegativas con mejores resultados (Grupo 6).

Conclusiones:

  • Los grupos de respuesta del huésped impulsados biológicamente pueden caracterizar eficazmente las infecciones agudas sospechosas.
  • Estos grupos identificados ofrecen un nuevo enfoque para estratificar a los pacientes y desarrollar estrategias terapéuticas específicas.
  • Comprender estas firmas de respuesta del huésped puede conducir a tratamientos más personalizados y efectivos para infecciones que requieren hospitalización.
