Resumen

La pandemia de COVID-19 afectó negativamente la función cardiopulmonar en pacientes con síndrome coronario agudo después de la PCI, con una absorción de oxígeno pico más baja observada durante la pandemia. Factores como la edad, el sexo, el IMC y la gravedad de la enfermedad influyeron en estos resultados, lo que requirió intervenciones personalizadas.

Sus antecedentes:

  • La pandemia de COVID-19 interrumpió significativamente los sistemas de salud y la gestión de pacientes a nivel mundial.
  • La gestión de las enfermedades cardiovasculares (ECV), especialmente para los pacientes con síndrome coronario agudo (SAA), se enfrentó a desafíos sin precedentes.
  • Comprender el impacto de la pandemia en la función cardiopulmonar posterior a la intervención es crucial para optimizar la recuperación del paciente.

Objetivo del estudio:

  • Evaluar los cambios en la función cardiopulmonar entre los pacientes con SCA después de la intervención coronaria percutánea (ICP) durante las diferentes fases de la pandemia de COVID-19.
  • Identificar los factores que influyen en la función cardiopulmonar en pacientes con SCA durante la pandemia.
  • Evaluar los efectos a largo plazo de la pandemia en los resultados de la rehabilitación cardiovascular.

Principales métodos:

  • Un estudio de cohorte transversal de un solo centro analizó a 7.650 pacientes con SCA sometidos a una revascularización PCI completa.
  • Los pacientes fueron estratificados en grupos pre-pandémicos, durante la pandemia y post-pandémicos basados en las fechas del estudio.
  • Para equilibrar las características basales se utilizó el Propensity Score Matching (PSM), con 4. 503 pacientes incluidos en el análisis final. Se compararon las pruebas de ejercicio cardiopulmonar (CPET) y los datos clínicos/laboratoriales utilizando ANOVA. El aprendizaje automático y la regresión logística identificaron las variables clave que influyen.

Principales resultados:

  • El pico de absorción de oxígeno por kilogramo (pico VO2/kg), un parámetro clave de CPET, fue significativamente menor en el grupo durante la pandemia (15,07 ml/min/kg) en comparación con los grupos pre-pandémicos (15,56 ml/min/kg) y post-pandémicos (15,69 ml/min/kg).
  • El análisis multivariado identificó la edad avanzada, el sexo femenino, el IMC más alto, el puntaje SYNTAX elevado, el aumento de NT-pro BNP, el aumento de HbA1c, el tabaquismo, la ansiedad y una mayor gravedad del SCA como factores asociados con la función cardiopulmonar deteriorada.
  • Estos hallazgos sugieren un impacto medible de la pandemia en la recuperación y rehabilitación cardiovascular.

Conclusiones:

  • La pandemia de COVID-19 se asoció con una disminución de la función cardiopulmonar entre los pacientes con ACS tratados con PCI.
  • Varios factores clínicos, demográficos y psicosociales estuvieron vinculados a peores resultados cardiopulmonares durante la pandemia.
  • Estos resultados subrayan la importancia de desarrollar estrategias adaptativas e intervenciones específicas para apoyar la rehabilitación cardiovascular durante las crisis de salud pública.
