Resumen

Un nuevo método de aprendizaje automático basado en el manguito reconstruye con precisión la presión del ventrículo izquierdo (LV) a partir de formas de onda braquiales no invasivas. Este enfoque ofrece una herramienta prometedora para evaluar la función cardíaca sin procedimientos invasivos.

Sus antecedentes:

  • La medición invasiva de la presión ventricular izquierda (LV) es el estándar de oro para la evaluación de la función cardíaca.
  • Los métodos actuales se basan en el cateterismo, lo que limita la accesibilidad y el uso clínico generalizado.

Objetivo del estudio:

  • Desarrollar un enfoque de aprendizaje automático no invasivo basado en el manguito (ML del manguito) para reconstruir la presión del LV.
  • Para permitir la evaluación de la función cardíaca en el lecho utilizando formas de onda braquial.

Principales métodos:

  • Entrenado un modelo de manguito-ML usando formas de onda braquial para predecir la presión LV.
  • Evaluación de la precisión del modelo con respecto a las mediciones invasivas por catéter en 104 sujetos.
  • Función cardíaca evaluada utilizando el derivado de la presión máxima del LV ([+]dP/dt).

Principales resultados:

  • Cuff-ML reconstruyó con precisión la forma de onda de LV (mediana de NRMSE = 8,2%).
  • Se observaron fuertes correlaciones para los parámetros de presión: presión máxima (r = 0,92), presión arterial media (r = 0,94), presión desarrollada (r = 0,85).
  • Se identificó una contractilidad sistólica anormal con una sensibilidad del 72% y una especificidad del 73%.

Conclusiones:

  • Cuff-ML reconstruye con precisión la presión LV de las mediciones no invasivas del manguito braquial.
  • Este método no invasivo muestra potencial para la evaluación de la función cardíaca al lado de la cama.
  • Se necesita más investigación para validar este enfoque prometedor.
