Sus antecedentes:

  • La inflamación sistémica, indicada por la proteína C reactiva de alta sensibilidad (hs-CRP), está relacionada con los resultados del accidente cerebrovascular.
  • La comprensión de la interacción entre la inflamación y el metabolismo de la glucosa en el accidente cerebrovascular isquémico agudo (AIS) es crucial para predecir el pronóstico del paciente.

Objetivo del estudio:

  • Investigar la relación entre los niveles basales de hs-CRP y los resultados clínicos en pacientes con EIA.
  • Analizar cómo diferentes estados del metabolismo de la glucosa (regulación normal de la glucosa, prediabetes, diabetes) modifican la asociación entre la hs-CRP y los resultados del accidente cerebrovascular.

Principales métodos:

  • Se utilizaron datos del Registro Nacional de Accidentes Cerebrovasculares III de China, incluidos 6.916 pacientes con EIA.
  • Los pacientes se clasifican en función de los niveles de hs-CRP (< 1,0 mg/ L, 1 a 3 mg/ L, > 3 mg/ L) y el estado de la glucosa definido por la HbA1c (NGR, pre- DM, DM).
  • Se empleó regresión logística multivariable y análisis de espina cúbica restringida para evaluar los resultados (estado funcional deficiente a los 3 meses y 1 año, recurrencia de accidente cerebrovascular).

Principales resultados:

  • El aumento de la hs-CRP (> 3 mg/ L) se asoció con un mayor riesgo de resultados funcionales deficientes a los 3 meses en pacientes con diabetes (aOR=1,38) y regulación normal de la glucosa (aOR=1,58).
  • No se observó una asociación significativa entre la alta hs-CRP y los malos resultados en el grupo prediabético (aOR=1,16).
  • Se observó una relación dosis-respuesta entre la hs-CRP y los resultados en la población total y en el grupo de prediabetes; no se encontraron asociaciones significativas para los resultados a 1 año o la recurrencia del accidente cerebrovascular.

Conclusiones:

  • El estado del metabolismo de la glucosa modifica significativamente el impacto de la hs-CRP en los resultados funcionales a corto plazo después de la AIS.
  • Un nivel alto de hs-CRP plantea un mayor riesgo de recuperación funcional a los 3 meses en pacientes con EIA con diabetes y regulación normal de la glucosa en comparación con la pre-diabetes.
