Resumen

La hematopoyesis clonal de mutaciones de potencial indeterminado (CHIP), particularmente TET2/ ASXL1, está relacionada con un aumento de la mortalidad y las características de la placa vulnerable en pacientes con infarto de miocardio por elevación del segmento ST (STEMI). Sus antecedentes:

  • La hematopoyesis clonal de potencial indeterminado (CHIP) está asociada con enfermedades cardiovasculares.
  • Las implicaciones pronósticas y patológicas de CHIP en el infarto agudo de miocardio siguen sin estar claras.
  • Este estudio investiga el CHIP en el infarto de miocardio por elevación del segmento ST (STEMI).

Objetivo del estudio:

  • Examinar la asociación entre las mutaciones CHIP (VAF 0,5% - 2%) y los eventos cardiovasculares adversos mayores (MACE) en pacientes con ITST.
  • Evaluar la relación entre el CHIP y las características de la lesión causante.
  • Explorar el impacto del CHIP en la eficacia del tratamiento.

Principales métodos:

  • La secuenciación profunda dirigida analizó las mutaciones CHIP en 1403 pacientes con STEMI.
  • Durante el seguimiento se registraron eventos cardiovasculares adversos importantes (EACM).
  • La tomografía de coherencia óptica (OCT) evaluó la morfología de las placas culpables en 355 pacientes.

Principales resultados:

  • La frecuencia de la mutación CHIP aumentó con la edad.
  • Las mutaciones TET2 o ASXL1 CHIP (VAF > 0,5%) se correlacionaron con una mayor mortalidad y ruptura de las tapas fibrosas en las lesiones culpables.
  • Los pacientes con CHIP mostraron mayores beneficios del tratamiento con ticagrelor y estatinas.
  • El CHIP combinado y la inflamación alta (hs-CRP > 5,8 mg/L) aumentaron significativamente el riesgo de mortalidad.

Conclusiones:

  • Las mutaciones TET2/ASXL1 CHIP (VAF > 2%) con alta inflamación predicen una alta mortalidad en el STEMI.
  • CHIP (VAF > 0,5%) está asociado con características de placa vulnerables en STEMI.
  • El estado CHIP influye en los resultados clínicos y la respuesta al tratamiento en pacientes con ITST.
