Resumen

La colecistitis, o inflamación de la vesícula biliar, puede aumentar causalmente el riesgo de colangiocarcinoma (cáncer del conducto biliar). Este estudio de aleatorización mendeliana sugiere que el tratamiento de la enfermedad de la vesícula biliar podría reducir potencialmente el riesgo de cáncer de conducto biliar.

Sus antecedentes:

  • La incidencia global de colangiocarcinoma (cáncer del conducto biliar) está aumentando, en particular el colangiocarcinoma intrahepático.
  • Los estudios epidemiológicos sugieren una relación entre la colecistitis (inflamación de la vesícula biliar) y el aumento del riesgo de cáncer hepatobiliar.
  • Los estudios observacionales tienen limitaciones para establecer la causalidad debido a factores de confusión.

Objetivo del estudio:

  • Investigar si la colecistitis es un factor de riesgo causal independiente para el colangiocarcinoma.
  • Utilizar la aleatorización mendeliana (MR) para superar las limitaciones de los estudios observacionales.

Principales métodos:

  • Se empleó la aleatorización mendeliana (MR) utilizando 25 polimorfismos de nucleótido único (SNP) como variables instrumentales para la colecistitis.
  • Se utilizó un gran conjunto de datos de estudios de asociación de todo el genoma (GWAS) de la Unidad de Epidemiología Integrativa (IEU) para los datos de resultados del colangiocarcinoma.
  • Aplicó cinco técnicas estadísticas de RM y realizó análisis de sensibilidad que incluyen la interceptación de RM Egger, el leave-one-out y los gráficos de embudo.

Principales resultados:

  • El método inverso ponderado por variación (IVW, por sus siglas en inglés) indicó una asociación significativa entre la colecistitis genéticamente predicha y el aumento del riesgo de colangiocarcinoma (OR=2,915, P=0,038).
  • El método de mediana ponderada apoyó este hallazgo (P=0,016).
  • Los análisis de sensibilidad (MR Egger, modo simple, modo ponderado) y las pruebas de pleiotropía no mostraron efectos de confusión significativos.

Conclusiones:

  • La evidencia apoya una relación causal entre la colecistitis y el colangiocarcinoma.
  • Los pacientes con colecistitis pueden tener un riesgo elevado de desarrollar colangiocarcinoma.
  • Los hallazgos pueden orientar las estrategias clínicas para el tratamiento de la colecistitis para reducir potencialmente la incidencia del colangiocarcinoma.
