Sus antecedentes:

  • La industria del cuidado de la salud está cambiando de una tarifa por servicio impulsada por el volumen a una atención responsable basada en el valor.
  • Los modelos de pago actuales no alinean adecuadamente la relación cirujano-paciente, lo que afecta el compromiso y los resultados del paciente.

Objetivo del estudio:

  • Revisar el cambio conceptual hacia la atención basada en valores en cirugía.
  • Aclarar los impulsores del rendimiento basado en el valor quirúrgico.
  • Esbozar una estrategia para participar en la reforma de la asistencia sanitaria.

Principales métodos:

  • Una búsqueda exhaustiva de la literatura de PubMed / MEDLINE / Google.
  • Inclusión de publicaciones sobre iniciativas basadas en valores, informes de agencias administrativas y gubernamentales y políticas de CMS/CMMI.
  • Revisión realizada desde el 1 de junio de 2024 hasta el 17 de mayo de 2025.

Principales resultados:

  • La atención basada en valores requiere una reevaluación de cómo los cirujanos definen y brindan atención.
  • Se introduce un nuevo marco tridimensional, EM3 (Experience Management cubed), para gestionar la experiencia del paciente.
  • EM3 se centra en los resultados clínicos, la participación del paciente y el gasto en episodios de atención para optimizar el valor (valor = resultados / costo).

Conclusiones:

  • El marco EM3 proporciona una hoja de ruta para operar la transparencia y mejorar el valor quirúrgico.
  • Mejora la competencia de la cadena de valor quirúrgica, el liderazgo basado en el equipo y la gestión de la atención longitudinal.
  • Las especialidades quirúrgicas deben desarrollar nuevas medidas de evaluación y planes de atención que incorporen EM3 para seguir siendo competitivas en los modelos de atención responsable.
