Resumen
Diagnosticar las infecciones por raquitismo en África es un desafío. Una nueva prueba de transcripción inversa de ARNr (RT-PCR) es prometedora para identificar estas infecciones, lo que sugiere el uso de doxiciclina para la enfermedad febril.

Objetivo del estudio:
- Evaluar la utilidad del ensayo de inmunofluorescencia (IFA) y la PCR de transcripción inversa de ARNr (RT-PCR) para el diagnóstico de infecciones ricettias agudas en Uganda.
- Evaluar las características clínicas asociadas con las infecciones por raquitismo en pacientes con enfermedad febril aguda.
Principales métodos:
- Se analizaron muestras de suero archivadas de las cohortes de sepsis y enfermedad febril aguda de Uganda.
- Las infecciones por rickettsial se detectaron mediante ensayo de inmunofluorescencia y PCR de transcripción inversa de ARNr.
- Se determinó la precisión diagnóstica de la RT-PCR de ARNr.
Principales resultados:
- Las infecciones por rickettsial se identificaron en el 10,0% de los 329 participantes.
- La RT-PCR de ARNr sérico demostró una sensibilidad del 75,0% y una especificidad del 91,2%.
- La trombocitopenia fue significativamente más frecuente en pacientes con infecciones ricettias (aOR 3,7).
Conclusiones:
- La PCR de transcripción inversa de ARNr es una herramienta valiosa para el diagnóstico de infecciones ricettias agudas en entornos con recursos limitados.
- Se recomienda el tratamiento empírico con doxiciclina para la enfermedad febril no malarial en esta región debido a la prevalencia de infecciones ricettias.
