Sus antecedentes:

  • La diabetes tipo 1 (DT1) y las enfermedades hepáticas autoinmunes (EIAH) implican daños mediados por el sistema inmunológico.
  • Los estudios de observación sugieren asociaciones, pero los vínculos causales entre la diabetes tipo 1 y las AILD (AIH, PBC, PSC) no están claros.
  • Los datos genéticos se utilizan para investigar estas relaciones complejas.

Objetivo del estudio:

  • Determinar las relaciones causales entre la diabetes tipo 1 y las enfermedades hepáticas autoinmunes utilizando evidencia genética.
  • Para investigar la causalidad bidireccional entre el T1D y las AILD.
  • Para evaluar la arquitectura genética compartida entre estas condiciones.

Principales métodos:

  • Análisis bidireccional de aleatorización mendeliana (MR) utilizando datos de un estudio de asociación de todo el genoma a gran escala.
  • Se utilizará el método inverso de variación ponderada (IVW) como método principal, con análisis de sensibilidad extensos (MR-Egger, mediana ponderada, etc.). para garantizar la robustez.
  • Análisis de correlación genética y colocalización para identificar variantes causales compartidas.

Principales resultados:

  • La DT1 predicha genéticamente aumenta causalmente el riesgo de hepatitis autoinmune (HAI).
  • Existe una probable relación causal bidireccional entre la DT1 y la colangitis biliar primaria (CBP).
  • No se encontró ningún vínculo causal genético entre la DT1 y la colangitis esclerosante primaria (CSP), a pesar de la correlación genética significativa.

Conclusiones:

  • Proporciona evidencia genética para el T1D que causa AIH y un vínculo bidireccional con PBC.
  • Hace hincapié en la necesidad de una vigilancia clínica y de estudios mecanicistas para las comorbilidades T1D-AILD.
  • Refine la comprensión de la interacción genética en las enfermedades autoinmunes.
