Sus antecedentes:

  • El COVID-19 presenta una mayor mortalidad y gravedad en los hombres en comparación con las mujeres.
  • Comprender los factores pronósticos como la vacunación, las comorbilidades y los marcadores inflamatorios es crucial para los pacientes hospitalizados.

Objetivo del estudio:

  • Comparar el impacto pronóstico de la vacunación, las comorbilidades y los biomarcadores inflamatorios en los resultados clínicos en pacientes hospitalizados con COVID-19, analizando las diferencias sexuales.
  • Investigar las diferencias específicas por sexo en la progresión de la enfermedad de COVID-19 y la respuesta al tratamiento.

Principales métodos:

  • Análisis de los parámetros clínicos y de laboratorio de 702 pacientes hospitalizados con COVID-19 (junio de 2020-abril de 2022).
  • Análisis comparativo basado en el sexo de los resultados clínicos, RT-qPCR y niveles de anticuerpos IgM e IgG específicos del SARS-CoV-2.
  • Evaluación de los biomarcadores inflamatorios (CRP, IL-6, etc.) y las enzimas hepáticas (AST, ALT, GGT).

Principales resultados:

  • Los machos mostraron niveles más altos de biomarcadores proinflamatorios y enzimas hepáticas, correlacionados con la gravedad del resultado clínico.
  • Carga viral más alta (valores más bajos de RT-qPCR Ct) correlacionados con niveles elevados de IL-6 en hombres.
  • Las comorbilidades fueron más frecuentes en las mujeres y los resultados graves; la vacunación se correlacionó con una menor gravedad, menores marcadores inflamatorios y un menor riesgo de neumonía.
  • La vacunación redujo la necesidad de corticosteroides pero aumentó la necesidad de tratamientos antivirales.

Conclusiones:

  • Los biomarcadores inflamatorios son indicadores críticos de la gravedad del COVID-19, especialmente en los hombres.
  • La vacunación reduce significativamente la gravedad y el riesgo de mortalidad por COVID-19, pero no ofrece una protección completa.
  • La terapia antiinflamatoria es importante para los pacientes vacunados, y las diferencias específicas por sexo en la progresión de la enfermedad requieren más investigación.
