Resumen

La pandemia de COVID-19 disminuyó la incidencia de fracturas de tobillo, pero aumentó las fracturas trimalleolares en las mujeres mayores. El tratamiento cambió a la fijación externa, con estadías hospitalarias más largas.

Objetivo del estudio:

  • Para analizar las tendencias en la incidencia de fracturas de tobillo, la demografía y el tratamiento de 2018-2023.
  • Para comparar los períodos pre-pandémico, pandémico y post-pandémico.
  • Para identificar cambios en los tipos de fracturas, las intervenciones quirúrgicas y la duración de la hospitalización.

Principales métodos:

  • Estudio observacional retrospectivo de 1010 casos de fractura de tobillo (2018-2023).
  • Datos recogidos de los registros médicos utilizando los códigos ICD-9-CM y las clasificaciones radiográficas (Danis-Weber, Lauge-Hansen, AO/OTA).
  • El análisis estadístico incluyó pruebas t, chi-cuadrado y regresión lineal (p < 0,05).

Principales resultados:

  • Se produjo una disminución del 31,7% en la incidencia de fracturas de tobillo en 2020.
  • Las fracturas trimalleolares aumentaron durante la pandemia, especialmente en mujeres mayores.
  • Las tasas de tratamiento quirúrgico aumentaron, con un cambio de ORIF a fijación externa; las estancias hospitalarias se alargaron.

Conclusiones:

  • La pandemia de COVID-19 alteró significativamente la epidemiología y el tratamiento de las fracturas de tobillo.
  • Se necesitan modelos de atención y estrategias preventivas adaptables, especialmente para los grupos vulnerables.
  • Los hallazgos subrayan el impacto duradero de la pandemia en el cuidado ortopédico del trauma.
