Resumen

La linfohistiocitosis hemofagocítica (HLH) en la infección por dengue es una afección grave que afecta principalmente a personas jóvenes. El reconocimiento temprano de la disfunción hepática y el tratamiento rápido son cruciales para mejorar las tasas de supervivencia en estos pacientes.

Sus antecedentes:

  • La linfohistiocitosis hemofagocítica (HLH) es un síndrome hiperinflamatorio que pone en peligro la vida.
  • La infección por el virus del dengue puede desencadenar HLH, presentando desafíos de diagnóstico debido a la superposición de síntomas con dengue grave y sepsis.

Objetivo del estudio:

  • Revisar y analizar los casos publicados de HLH asociados al dengue.
  • Identificar las características clínicas, los predictores de mortalidad y los resultados en la HLH asociada al dengue.

Principales métodos:

  • Se realizó una revisión narrativa y un metanálisis de los datos de cada paciente.
  • Buscado en las bases de datos de PubMed y Scopus hasta el 5 de marzo de 2025.
  • Se han extraído y analizado datos clínicos, de laboratorio, de tratamiento y de resultados de los estudios elegibles.

Principales resultados:

  • Se incluyeron 133 pacientes de 71 estudios; la edad media fue de 18 años.
  • Características comunes: fiebre, citopenias, organomegalia y disfunción hepática. Elevación de la ALT, ictericia, hipofibrinogenemia asociada con la mortalidad.
  • DENV-1 fue el serotipo más común; el 19,3% de los pacientes falleció. No se han identificado predictores de mortalidad independientes.

Conclusiones:

  • La HLH asociada al dengue afecta significativamente a los individuos jóvenes con una alta mortalidad.
  • La disfunción hepática, el shock o la insuficiencia de órganos indican un mal pronóstico.
  • El reconocimiento temprano y el tratamiento inmunomodulador, como los corticosteroides, pueden mejorar los resultados.
