Sus antecedentes:

  • Los pacientes con oclusiones ateroscleróticas sintomáticas de la arteria carótida interna (ICA) o de la arteria cerebral media (MCA) tienen un alto riesgo de recidiva de ictus isquémico.
  • Los ensayos anteriores no sugirieron ningún beneficio de la cirugía de derivación extracraneal-intracraneal (EC-IC), posiblemente debido a la falta de potencia.

Objetivo del estudio:

  • Evaluar la eficacia de la cirugía de derivación EC-IC en la reducción del riesgo de accidente cerebrovascular o muerte en pacientes con oclusiones sintomáticas de ICA o MCA.
  • Para analizar los datos de ensayos aleatorizados que comparan el bypass EC-IC más la terapia médica frente a la terapia médica sola.

Principales métodos:

  • Revisión sistemática de ensayos aleatorizados de PubMed, Embase y la Biblioteca Cochrane.
  • Metaanálisis utilizando datos de pacientes individuales reconstruidos (en una etapa y en dos etapas) para calcular los ratios de riesgo (RR).
  • Resultado primario: composición de ictus o muerte dentro de los 30 días, o ictus isquémico ipsilateral más allá de los 30 días hasta 2 años.

Principales resultados:

  • Se incluyeron en el metanálisis cuatro ensayos con 2102 participantes.
  • No se encontró ninguna diferencia estadísticamente significativa en el resultado compuesto primario entre el grupo de bypass EC- IC y el grupo de terapia médica en solitario (HR de una etapa 1.019, P=0.06; HR de dos etapas 0.954, P=0.21).
  • Las tasas de accidente cerebrovascular isquémico ipsilateral, cualquier accidente cerebrovascular o muerte, y cualquier accidente cerebrovascular fueron similares entre los grupos a los 2 años.

Conclusiones:

  • La adición de cirugía de derivación EC-IC a la terapia médica no reduce significativamente el riesgo de resultado compuesto de accidente cerebrovascular o muerte en pacientes con oclusiones sintomáticas de ICA o MCA.
  • Los hallazgos sugieren que la terapia médica actual por sí sola es tan efectiva como agregar una cirugía de derivación para la prevención del accidente cerebrovascular en esta población de pacientes.
