Hepatitis is an inflammatory condition of the liver most commonly caused by hepatotropic viruses (A–E), though non-infectious causes such as alcohol and drugs also exist.Hepatitis AHepatitis A virus (HAV) is a non-enveloped RNA virus of the Picornaviridae family. It is primarily transmitted via the fecal-oral route, typically through ingestion of contaminated food or water. After ingestion, HAV enters the bloodstream through the oropharynx or intestinal epithelium and reaches the liver.

Resumen

Los modelos de aprendizaje automático identifican efectivamente a las personas con alto riesgo de infección por el virus de la hepatitis C (VHC). El modelo Gradient Boosting Machine demostró un rendimiento superior, ofreciendo una herramienta prometedora para la detección específica del VHC en entornos clínicos.

Sus antecedentes:

  • Las infecciones por el virus de la hepatitis C (VHC) están aumentando en los Estados Unidos, exacerbadas por la epidemia de opiáceos.
  • Una parte significativa de las infecciones por el VHC permanecen sin diagnosticar debido a la presentación asintomática.
  • Hay una necesidad crítica de herramientas de detección eficaces para identificar a las poblaciones en riesgo.

Objetivo del estudio:

  • Desarrollar y validar algoritmos de aprendizaje automático para predecir y estratificar el riesgo de infección por el VHC.
  • Crear una herramienta que pueda ayudar en las estrategias de detección específicas del VHC.

Principales métodos:

  • Utilizó la base de datos de registros electrónicos de salud OneFlorida+ para el período 2016-2023.
  • Se incluyeron individuos mayores de 18 años sometidos a pruebas de detección del VHC, analizando 275 características.
  • Desarrollado y validado cuatro modelos de ML: red elástica, bosque aleatorio, máquina de impulso de gradiente y red neuronal profunda.

Principales resultados:

  • El modelo Gradient Boosting Machine (GBM) logró el rendimiento más alto (estadística C: 0,916).
  • GBM demostró una sensibilidad del 79,39% y una especificidad del 89,08%, identificando un caso positivo de VHC por cada seis pruebas.
  • Los tres primeros deciles de riesgo fueron capturados en más del 90% de los pacientes con VHC.

Conclusiones:

  • Los algoritmos ML proporcionan un método eficaz para predecir y estratificar el riesgo de infección por el VHC.
  • Estos modelos representan una herramienta prometedora para el cribado del VHC en la práctica clínica.
  • El estudio pone de relieve el potencial de los enfoques basados en datos en la gestión de desafíos de salud pública como el VHC.
