Objetivo del estudio:

  • Investigar las asociaciones entre la flexibilidad de las habilidades de ayuda del terapeuta (HSF), la alianza de trabajo (WA), la evaluación de la sesión (SE) y el funcionamiento del cliente.
  • Examinar el efecto indirecto de HSF en la mejora de los clientes, con la mediación de WA y SE.
  • Para explorar la precedencia temporal y la causalidad en estas relaciones dentro de la terapia psicodinámica.

Principales métodos:

  • Los pacientes adultos (n=121) en terapia psicodinámica calificaron su funcionamiento, WA, SE y el terapeuta HSF.
  • Los datos se recopilaron al comienzo y al final de cada sesión de tratamiento.
  • Los análisis estadísticos examinaron las asociaciones y los efectos de mediación, controlando posibles factores de confusión como la duración del tratamiento y la causalidad inversa.

Principales resultados:

  • El terapeuta HSF en una sesión se asoció positivamente con las calificaciones posteriores de los clientes de WA y SE.
  • Se encontró un efecto indirecto significativo del HSF del terapeuta en el funcionamiento posterior del cliente, mediado por el WA, pero no por el SE.
  • No se observó evidencia de moderación por la duración del tratamiento, efectos cuadráticos o causalidad inversa.

Conclusiones:

  • El HSF del terapeuta parece preceder temporalmente e influir plausiblemente en las mejoras en la alianza de trabajo.
  • La alianza de trabajo, a su vez, precede temporalmente y presumiblemente conduce a mejoras en el funcionamiento del cliente.
  • Estos hallazgos ponen de relieve la importancia de la capacidad de adaptación del terapeuta para fomentar una fuerte alianza terapéutica y promover el progreso del cliente.
